MTI scheme closure

Following a review of the Medical Training Initiative (MTI), NHS England has decided that the current model is no longer consistently meeting its objectives and is therefore beginning the process of winding down this programme. This means that we can no longer accept new applications to the MTI scheme as of 31 March 2026.

The College will continue supporting all MTI doctors already on the scheme in anaesthesia/Intensive Care/Pain Medicine, to complete their MTI placements as planned. 

Applications already received by the College and in progress before 12:00 noon on 31 March 2026 will continue to be processed. The deadline to complete all required steps in the College application process including the AoMRC application process is the end of the day 30 June 2026.

The Medical Training Initiative (MTI) is a government scheme designed to assist doctors with proven ability in anaesthesia/intensive care/pain Medicine from low and middle-income countries (lower-middle-income and upper-middle-income countries), according to World Bank Definitions, to pursue further specific specialist training in the UK. Doctors can come to the UK for up to 24 months under the UK Border Agency’s Temporary Work – Government Authorised Exchange (formerly Tier-5) visa. It is hoped that these individuals will return and take on leadership roles in their home hospitals, especially in the areas of Quality Improvement and Training.

Global Fellowship Scheme 

The Global Fellowship Scheme (GFS) is a sponsorship scheme run by the RCoA open to anaesthetists, Intensive Care Medicine and Pain Medicine doctors who work and train in High Income Countries (HICs) to undertake a period of subspecialty training within the NHS. Successful applicants will be sponsored by the employing Trust (as outlined through immigration rules).

We also work with health providers in some countries to provide opportunities for local UK Anaesthetist in Training to train in low and middle-income countries.

We collaborate with health ministries (or equivalent) and professional organisations to support our aim of strengthening global healthcare systems.
