Curriculum change form for the Lifelong Learning Platform

This form is for users of the Lifelong Learning Platform who wish to request a change of assigned curriculums.

If you are moving from a 2010 curriculum to a 2021 curriculum, you will need to have approval from any one of the following: Head of School, Training Programme Director, Regional Adviser or Deputy Regional Adviser. We may ask you to provide evidence of this approval (an email confirmation from one of the above).

This form will be processed in two working days or by the deadline requested where applicable.
